How to apply for an NPI Type 2

If you are a mental health therapist running your practice as an LLC, you will almost certainly need an NPI Type 2. This guide walks through exactly how to apply for an NPI Type 2, what information you need, and common mistakes therapists should avoid.

Step 1: Go to the NPPES Website

Visit the official NPPES site: https://nppes.cms.hhs.gov/

Log in using your existing individual NPPES account.

Step 2: Choose “Apply for an NPI for an Organization”

Once logged in:

  • Navigate to the section that allows you to apply for a new NPI

  • Select Apply for an NPI for an Organization

  • Select I am an Employee of the Organization

This is the key step. Many therapists accidentally apply for another individual NPI instead of an organizational one. Make sure you are clearly in the organization flow before continuing.

Step 3: Complete the Organization Application

You will be asked for information about the practice, not about you personally.

You should include:

  • LLC legal name exactly as registered

  • Practice location and mailing address

  • EIN

  • Contact information for the organization

  • Authorized official details

Important: Make sure to NOT answer with personal details. Answer it from the point of view of the company. For example, do Not Enter Personal SSN Information

Step 4: Review and Submit

Once submitted, most Type 2 NPIs are issued within a few business days, often faster. Confirmation should come through email.
